What is care management and what does a care manager do?

Care managers are Master's level trained professionals who specialize in assessing needs and risk of people living with an illness. Care managers have extensive knowledge about medical resources and they are concerned with quality standards of patient care. Care Managers also have relationships with agencies that provide resources and they may make recommendations and referrals for services, on behalf of their clients. Care managers assist when family members are unavailable to help to manage daily living needs.
